Proof of residency required:

Must have a valid Utah driver’s license plus a copy of one of the following:

  • Property Tax form
  • Bill to primary residence (e.g., Heber Light and Power, Water, Sanitation, Sewer, Phone, etc.)

Midway Cemetery Veteran’s Flag Project

Starting in 2023 Midway City has taken over the placement of veteran’s flags on the gravesites of the deceased and honored veterans buried in the Midway City Cemetery. To make sure all our veterans are honored we would like to have your input so that we can improve our veteran burial information. Please fill out the following form regarding any veteran that you know is buried in the Midway City Cemetery.
